Instrument pre-soak is a critical first step in the dental instrument reprocessing cycle. It involves keeping soiled instruments moist immediately after a procedure to prevent organic matter from drying and hardening, which would otherwise compromise the sterilization process.

Description

An instrument pre-soak is a specialized solution—often enzymatic or humectant-based—designed to break down bioburden (blood, saliva, and tissue) at the point of use. In a busy dental practice where immediate cleaning is not always possible, these solutions act as a “holding” agent. They are available as liquids for immersion or sprays and foams that coat the instruments.

Key Features

  • Enzymatic Action: Most pre-soaks contain enzymes like protease (breaks down proteins), amylase (starch), and lipase (fats) to chemically digest organic debris.
  • Humectant Properties: Formulated to retain moisture for extended periods, preventing the formation of a “hardened skin” on the instruments that can shield microbes from the autoclave.
  • Corrosion Inhibition: Contains protective agents to prevent pitting, rusting, and staining while instruments are submerged.
  • Neutral pH: Usually formulated with a pH of around 7 to be safe for a wide variety of materials, including stainless steel, plastics, and delicate hinged instruments.
  • Non-Aerosol/Low Foaming: Designed to be safe for the staff to apply and compatible with subsequent steps like ultrasonic cleaning or automated washers.

Usage Instructions

  1. Immediate Application: Apply the pre-soak as soon as the procedure is finished. For sprays like Prepclean Forever Wet, coat the instruments thoroughly while they are still at the chairside.
  2. Submergence: If using a liquid soak, ensure all instruments are fully submerged in a dedicated holding container.
  3. Dilution: If the product is a concentrate, such as All-in-one Enzymatic Cleaner, follow the manufacturer’s ratio (e.g., 1 oz per gallon) using lukewarm water.
  4. Timing: Instruments can typically stay in a pre-soak for 10 to 20 minutes, or even overnight/weekends if using specific “long-lasting” humectant formulas.
  5. Rinsing: Always rinse instruments thoroughly with water after the pre-soak and before moving them to the ultrasonic cleaner or manual scrubbing stage.
